Hart Feldt is a small, family-based NDIS provider dedicated to empowering people with disabilities through personalised support.

Our unique name is a blend of our maiden surnames that creates exactly the type of services we offer, and the ethos that drives us: Heart felt.

Our team brings together experienced professionals from diverse backgrounds giving you real choice and control in how you're supported, every step of the way.

Laura holds a Bachelor’s Degree in Social Sciences, specialising in Mental Health, and Jo holds a Bachelor’s Degree in Education.

As a mother-daughter duo, they’ve combined their expertise to create a service that’s compassionate, person-centred, and deeply rooted in both personal and professional experience.

Alyssa is an experienced, organised and tech-savvy support worker with exceptional interpersonal skills. She is passionate about going the extra mile for participants and loves animals. Alyssa holds a Degree in Health Science.

Sabrina is a highly experienced and compassionate support worker. She is bilingual and speaks fluent Spanish and English. Sabrina enjoys travelling and sharing her Brazillian culture through food

With a Bachelor of Policing, Tristan is a strong skills in advocacy and a deep understanding of human rights. He enjoys supporting participants to engage meaningfully in their communities and assisting with legal matters, including court attendances and navigating legal processes.

Missha is an experienced support worker with a Certificate IV in Individual Support. Missha is known for her kindness and willingness to work with participants and learn new things together. Missha is bilingual, speaking fluent English and Nepalese.

Our Dedicated Support Team

Bibek is an Enrolled Nurse with over 5 years’ experience in Support Work. He is bilingual in Nepalese and fluent in English. Bibek is a keen photographer and is known for his empathetic and compassionate approach.

Zoe is a warm and empathetic support worker and an animal lover. She is highly experienced, having worked in the disability sector for five years, and she is a confident advocate for her participants. Zoe holds a Masters of Public Health.

Jordy is a dedicated support worker currently completing a Bachelor of Teaching. She is passionate about supporting children and young people and plans to continue in the disability sector.

Susan is an experienced Disability Support Worker and qualified swim coach. She brings a calm, practical, and genuine approach to her work.

With a Certificate IV in Mental Health and lived experience, Angela supports participants to live meaningful and vibrant lives. She is known for building trusted, reliable relationships

Aadarsha is an experienced support worker with a Certificate IV in Individual Support. He is bilingual, speaking fluent English and his native Nepalese. Aadarsha has wide ranging interests from sports to cars that he enjoys sharing with participants.
