At Hart Feldt NDIS Services, we welcome enquiries and referrals from NDIS participants, families, carers, Support Coordinators, healthcare professionals, and community organisations.

We understand that starting NDIS services or transitioning from another provider can feel overwhelming, which is why our onboarding process is designed to be simple, efficient, and supportive.

Once we receive your referral or enquiry, a member of our Operations Team will contact you, your nominee, or your carer to discuss your individual circumstances and determine how we can best support you. During this initial conversation, we will gather some basic information and explain the next steps in the onboarding process.
To commence services, we may ask you to complete the necessary documentation, including a Service Agreement and, where applicable, a Consent to Share Information form in accordance with NDIS requirements.
A Smooth Transition to Our Services
We recognise that navigating the NDIS can involve significant paperwork and coordination. Our experienced team is committed to making this process as straightforward and stress-free as possible.
Our Operations Manager, Support Workers, and administrative team work collaboratively with your Support Coordinator and other key stakeholders to ensure your transition is well planned and tailored to your needs. Together, we will develop a personalised support plan that reflects your goals, preferences, and desired outcomes.
Whether you require supports to commence immediately or prefer a gradual transition, we will work at your pace. We also assist with provider handovers, communication between service providers, and ongoing coordination to ensure continuity of care and a seamless experience.
