Hart Feldt NDIS Services provides both traditional NDIS supports and specialised services for individuals living with complex psychosocial disabilities and mental health challenges.
Our experienced team understands that when someone is in crisis or their support needs change, timely and responsive assistance can make all the difference. That's why we've developed our Fast-Start Support Pack, allowing participants to access Support Coordination and Core Supports quickly, with minimal paperwork and unnecessary delays.
To make the transition as smooth as possible, every new participant is supported by our unique two-person onboarding team. This approach provides continuity, builds trust over time, and gives participants and their support networks two dedicated points of contact while we gradually gather the information needed to tailor ongoing supports.

Our Core Supports include assistance with daily living, household tasks such as cleaning and yard maintenance, community participation, recreational activities, transport, and nursing care.
