Hart Feldt NDIS Services is committed to providing responsive, timely, competent and appropriate supports that meet each participant’s needs, desired outcomes and personal goals. We understand that beginning services or transitioning from another provider can feel overwhelming, so our onboarding process is designed to be efficient, straightforward and as stress free as possible. Our first priority is to have your supports in place, your way.
A Request for Referral can be made by phone on 0481 504 684, by email at info@hartfeldt.org, or by completing the form on our Contacts page. Our Operations Team will then make contact with you and/or your carers to ensure we are the right fit for your circumstances. We will require some brief information and consents from you to get started. This typically includes a Service Agreement and a Consent to Share Information (NDIS Standard).
We know the NDIS paperwork can feel endless, so our approach is patient and steady: our friendly Operations Manager and Support Workers will follow up over time at your own pace. Support Coordination, Administration and Support Workers work together to provide you with a coordinated and comprehensive team. Together with your Support Coordinator, a Care Plan will be developed and supports will be engaged on your timeline. This can be organised as quickly — or as slowly — as you need while you transition to new services and supports. Our professional team will manage handovers and help you understand processes with clarity and transparency to ensure a smooth, low-stress transition.
