Hart Feldt NDIS Services offers traditional supports in addition to specialised services for complex psychosocial mental health. Our highly experienced staff understand the value of speed and accessibility when managing crises. We offer a Fast-Start Support Pack to onboard and transition participants experiencing fluctuating capacity with a minimum of disruption. Services can be activated quickly, including Support Coordination and Core Supports, relying on some simple details, without the burden of paperwork. Our unique two-person onboarding team ensures that the remainder of your information is captured over a period of time as trust is built, giving you two points of contact when communicating with Hart Feldt.

Core Supports

Our Core Support Services offers a wide range of practical assistance, including support with daily living tasks; domestic services such as cleaning and yard maintenance; recreational activities and creative community participation opportunities; as well as nursing care and reliable transportation. Our support workers are trained, experienced and compassionate. We carefully match the right team to your individual needs to promote inclusion, build independence and strengthen connections within the community.


Capacity Building

Reaching your goals is as important to us as it is to you. At Hart Feldt, we will support you to improve mobility, independence and your daily living skills through personalised, practical assistance. Our dedicated and experienced staff deliver recovery coaching programs designed to help you develop essential life skills and strategies for individual growth, with progress that can be clearly measured in the quality of life you choose to lead. Hart Feldt NDIS Services also provides opportunities for collaboration and socialisation within our network and beyond, helping you build friendships, increase confidence and participate more fully in the wider community.

Specialised Support Coordination

Our Support Coordinators are university-qualified Social Workers with extensive experience across the NDIS and Mental Health sectors. We are deeply committed to working collaboratively with participants, their families, carers, and allied health professionals to ensure that supports are thoughtfully tailored to each individual’s unique circumstances and goals. Our approach emphasises flexibility and responsiveness, recognising that needs can change over time and that ongoing engagement is essential to achieving positive outcomes. Regular communication, proactive planning, and coordinated review form the foundation of our service delivery, ensuring every participant receives the guidance, advocacy, and practical resources needed to thrive.

Recovery Coaching

Our qualified Social Workers will carefully develop and prepare individualised programs to facilitate Recovery Coaching, supporting participants step by step as they work toward their NDIS and personal goals. These programs are created in collaboration with you, designed to improve engagement with services that may have been previously overlooked, to provide steady support for participants and their families during times when mental health capacity fluctuates, and to help reduce the likelihood of hospital admissions. In addition, Hart Feldt NDIS Services provides Counselling Services that can be accessed either face to face or via telehealth to suit your preferences and needs.
