Hart Feldt is a small, family-based NDIS provider dedicated to empowering people with disabilities through personalised support. Our unique name is a blend of our maiden surnames that creates exactly the type of services we offer, and the ethos that drives us: Heart felt.
Our team brings together experienced professionals from diverse backgrounds giving you real choice and control in how you're supported, every step of the way.
Laura holds a Bachelor’s Degree in Social Sciences, specialising in Mental Health, and Jo holds a Bachelor’s Degree in Education.
As a mother-daughter duo, they’ve combined their expertise to create a service that’s compassionate, person-centred, and deeply rooted in both personal and professional experience.

Our Services
Hart Feldt NDIS Services offers traditional supports in addition to specialised services for complex psychosocial mental health. Our highly experienced staff understand the value of speed and accessibility when managing crises. We offer a Fast-Start Support Pack to onboard and transition participants experiencing fluctuating capacity with a minimum of disruption. Services can be activated quickly, including Support Coordination and Core Supports, relying on some simple details, without the burden of paperwork. Our unique two-person onboarding team ensures that the remainder of your information is captured over a period of time as trust is built, giving you two points of contact when communicating with Hart Feldt.
Core Services
Our Core Support Services offers wide ranging assistance, including support with daily living; domestic services including cleaning and yard maintenance; recreational activities and innovative community participation opportunities, nursing and transportation.
Our support workers are trained and experienced. We work on matching the best team to your needs to promote inclusion and strengthen community ties.

Capacity Building
Reaching your goals is as important to us as it is to you. At Hart Feldt, we will support you to improve mobility, independence and your daily living skills. Our dedicated and experienced staff support recovery coaching programs to enable you to develop essential life skills and strategies for individual growth that is measurable in the quality of life that you choose to lead.
Hart Feldt NDIS Services offers opportunities for collaboration and socialisation within our network to build friendships and engage in the wider community.
Specialised Support Coordination
Our Support Coordinators are university qualified Social Workers with vast experience in the NDIS and Mental Health sectors.
We are committed to working collaboratively with participants, their families, and allied professionals to ensure that supports are tailored to each individual's unique situation. Our approach emphasises flexibility and responsiveness, recognising that needs may change over time and that ongoing engagement is essential for positive outcomes. Regular communication and proactive planning form the foundation of our service delivery, ensuring that every participant receives the guidance and resources needed to thrive.
Psychosocial Recovery Coaching and Counselling Services
Our qualified Social Workers will develop and prepare individualised programs to facillitate Recovery Coaching to assist participants in reaching their NDIS and personal goals. Our programs are developed with and for you to enhance engagement in services that may have been previously neglected, to support participants and their families when mental health capacity fluctuates and to minimise the risk of hospital admissions.
In addition, Hart Feldt NDIS Services offers Counselling Services that may be accessed face to face or by telehealth.
Getting started with Hart Feldt NDIS Services
Hart Feldt NDIS Services is committed to the provision of responsive, timely, competent and appropriate supports to meet participant needs, desired outcomes and goals. We understand that beginning services or transitioning from another provider can feel overwhelming, so our onboarding process is designed to be efficient, easy and stress free. Our first priority is to have your supports in place, your way.

A Request for Referral can be made by phone on 0481 504 684, by email - info@hartfeldt.org, or by completing the form in our Contacts page.
Our Support Coordinator will make contact with you and / or your carers, to ensure we are the perfect fit.
We will require some brief information and consents from you to get started. This includes a Service Agreement and a Consent to Share Information (NDIS Standard)
Of course, the NDIS paperwork is endless, but our approach is to follow up over a period of time with our friendly Administrator at your own pace. Support Coordination, Administration and Support Workers function together to provide you with a comprehensive team.
Together with your Support Coordinator, a Support Plan will be developed and supports will be engaged.
This process can be completed in a matter of hours.
